1. Agreement to Terms

By accessing our website or utilizing the services provided by Eureka Care Providers Pty Ltd ("Eureka Care", "we", "us"), you agree to be bound by these Terms of Service. If you do not agree with any part of these terms, please do not use our services.

2. Service Provision

Eureka Care Providers is a Registered NDIS Provider delivering clinical nursing, personal care, community participation, and household support services. All services are delivered in accordance with standard NDIS guidelines and documented in a formal Service Agreement established between Eureka Care and the participant (or their representative) prior to the commencement of supports.

3. Participant Rights and Responsibilities

As a participant utilizing our services, you have the right to respect, dignity, privacy, and the provision of safe, high-quality care. In turn, you agree to:

Provide accurate and complete information regarding your health, support needs, and NDIS plan.
Communicate any changes in your condition or circumstances promptly.
Treat our staff and support workers with respect and provide a safe environment when services are delivered in your home.

4. Fees and Cancellations

Our fees align with the latest NDIS Pricing Arrangements and Price Limits. Cancellations must be made in accordance with the NDIS cancellation policy. Failure to provide adequate notice (typically 7 days) may result in cancellation fees being charged to your NDIS plan as per commission guidelines.

5. Limitation of Liability

While we strive to provide excellent care, Eureka Care Providers shall not be liable for any indirect, incidental, special, consequential or punitive damages resulting from your use of our services, except where liability cannot be excluded under Australian Consumer Law or the NDIS Act.

6. Amendments

We reserve the right to update or modify these terms at any time. Changes will be effective immediately upon posting to this website. Your continued use of the website and services constitutes your acceptance of the revised terms.
